UnitedHealthcare is seeking an RN Clinical Care Coordinator to manage a panel of members with complex needs. The role focuses on medical, behavioral, and social determinants of health to improve outcomes.

It is a field-based position with a home-based office and travel up to 75%, supporting care transitions and coordinating with care teams. Residents in Plymouth County, MA or surrounding areas may work remotely part-time while visiting members in the community.

Qualifications

• High School Diploma or GED required.
• Current and unrestricted RN license in MA.
• 2+ years of clinical experience.
• Proficient with MS Office (Word, Excel, Outlook).
• Access to a designated quiet home workspace with PHI protection.
• Must have reliable high-speed internet and ability to travel within territory.
• Valid US driver’s license.

Responsibilities

• Conduct comprehensive needs assessments across medical, behavioral, functional, cultural, and socioeconomic domains.
• Develop and implement person-centered care plans.
• Collaborate with internal care teams, providers, and community resources.
• Provide education and coaching to promote self-management.
• Coordinate care transitions post-ER, inpatient, or SNF.
