Job summary

UnitedHealth Group is seeking a Clinical Care Coordinator in Western Colorado (Mesa County, CO). This home-based role blends telework with 50–75% field work, coordinating medical, behavioral, and social needs to improve quality of care.

You’ll partner with care teams, providers, and community resources to implement person-centered care Plans and resolve barriers to timely care. Remote-friendly, competitive hourly pay plus benefits.

Qualifications

• Must meet one of the following: Current, unrestricted independent licensure as a Social Worker
• 4-Year or Higher degree in a behavioral health related field plus 3+ years of direct experience
• LPN with 5+ years of direct experience
• 2+ years of experience working within the community health setting or in a healthcare role
• 1+ year of experience with local health providers and/or community support organizations addressing the SDoH
• 1+ years of experience with MS Office, including Word, Excel, and Outlook
• Resident of Colorado
• Driver’s License and access to reliable transportation
• Preferred Qualifications: Motivational Interviewing, Stages of Change, Trauma-Informed Care, Person-Centered Care
• Experience in team-based care
• Experience with co-occurring disorders
• Bilingual in Spanish or other market language
• Background in Managed Care

Responsibilities

• Engage members and/or their families to complete a comprehensive needs assessment
• Develop and implement individualized, person-centered care plans
• Partner and collaborate with internal care team, providers, and community resources/partners
• Provide referral and linkage as appropriate
• Work with community partners to support members with complex needs
