Job summary

Vail Health is seeking a Triage RN to perform care coordination and professional nursing duties. You will assess patient health concerns using established triage protocols to determine dispositions to emergency care, urgent care, or clinic visits within licensed practice.

You will triage patient calls and portal messages as part of a care coordination team, document in the EHR, participate in KPI huddles, and support quality improvement initiatives to enhance access and outcomes across our care

Qualifications

• 3 years of clinical experience with patient care and administrative duties preferred.
• Nursing assessment skills with/without visual inspection using basic nursing theory.
• Knowledge of medical terminology, medical equipment, HIPAA compliance and OSHA rules.
• State RN license or compact license required.

Responsibilities

• Evaluates and assesses patient health care needs based on signs and symptoms using triage protocols.
• Participates in weekly huddles to review KPI and determine best clinical actions.
• Accurately documents symptoms, assessments, advice, and patient responses in the health record.
• Follows policies and protocols to ensure consistent care and improve health outcomes.
• Supports quality improvement and CPC+ Care Coordination team processes.
• Ensures relevant medical information is forwarded to proper departments for continuity of care.
• Documents in the EHR triage assessment, plan, implementation, and follow-up.
