Vail Health is seeking a Triage RN to perform care coordination and triage patient health concerns using established protocols. You will support providers with triaging calls and portal messages, and work within a clinical team to streamline care pathways.

Responsibilities include documenting assessments in the EHR, coordinating same-day and future appointments, and ensuring adherence to policies and KPI targets.

Qualifications

• 3 years clinical experience with applicable patient care and administrative duties preferred.
• Knowledge of disease processes and development across all age groups.
• Nursing assessment skills with/without visual inspection using basic nursing theory.

Responsabilités

• Evaluates and assesses patient health care needs using triage protocols.
• Participates in weekly huddles to review KPI and ensure KPI-aligned workflows.
• Coordinate and schedule same-day and future appointments on provider schedules.
• Document symptoms, nursing assessments, advice, and patient responses in the EHR.
• Follow policies and protocols to improve health care outcomes and access to care.
