Job summary

Vail Health is seeking a Triage RN to perform care coordination and triage nursing functions. You will assess health concerns using established protocols, triage patient calls and portal messages, and document assessments in the EHR.

You will participate in KPI reviews and CPC+ care coordination activities to improve patient access and outcomes. The role requires 3 years of clinical experience, licensed as an RN in the US (Colorado or compact state).

Qualifications

• 3 years clinical experience with applicable patient care and administrative duties preferred.
• Nursing assessment skills with/without visual inspection utilizing basic nursing theory required.
• Knowledge of medical terminology, medical equipment, HIPAA compliance and OSHA rules and regulations required.
• State of Colorado Registered Nurse or valid compact state license required.

Responsibilities

• Assess patient health needs using triage protocols and provide appropriate dispositions to emergency care, urgent care or clinic appointments.
• Triage calls and portal messages; support providers in triaging patient needs.
• Document symptoms, nursing assessment, advice given and patient responses in the EHR.
• Participate in KPI huddles and CPC+ Care Coordination team processes.
• Contribute to quality improvement efforts and continuity of care through proper information forwarding.
