CVS Health is seeking a dedicated Care Coordinator to support members with long-term rehabilitation and psychosocial needs from a home-based setting with 25–50% travel in Wayne and Macomb Counties. You will manage case plans, coordinate services, and provide psychosocial support to improve outcomes and reduce hospital readmissions.

The role requires a Master's in Social Work with LMSW licensure, strong communication, and digital literacy.

Qualifications

Master's Degree in Social Work required and LMSW licensure.
Minimum years of experience in medical social work or case management.
Willingness to travel up to 25–50% of the time in Wayne and Macomb Counties.
Strong problem-solving, communication and digital literacy skills.
Bilingual in Spanish or Arabic preferred.

Responsibilities

Coordinate care for members with long-term rehabilitation needs to prevent readmissions.
Perform initial and periodic assessments for long-term care or case management programs.
Develop individualized care plans and coordinate services across the continuum of care.
Communicate updates with providers and care teams to optimize outcomes.
Provide information and resources on social issues and mental health to members and families.
Mentor junior staff in social work techniques and processes.
