Job summary

CVS Health is seeking a Care Coordinator to manage long-term rehabilitation and psychosocial needs, coordinating case management and community resources to prevent readmissions and ensure quality care. This fully remote role includes 25%–50% travel within Wayne and Macomb Counties.

You will assess members, develop individualized plans, and collaborate with providers to optimize outcomes. A Master’s in Social Work and LMSW are required, with 40 hours weekly and full-time status.

Qualifications

• Minimum years experience in medical social work or case management.
• Must be willing to travel 25%–50% in Wayne and Macomb Counties.
• Basic knowledge of medical terminology and digital literacy.

Responsibilities

• Coordinate care for members with long-term rehabilitation or psychosocial needs to prevent readmissions.
• Implement and coordinate case management, psychosocial, and quality of life activities.
• Develop and carry out strategies to integrate acute and long-term care services.
• Communicate updates with providers, PCPs, Medical Directors, and care management staff.
• Identify opportunities to ensure members receive needed care under their benefit plan.
