Qualifications

Responsibilities

• Coordinate care for members with long-term rehabilitation and psychosocial needs to prevent readmissions or custodial placement.
• Perform initial and periodic assessments for long-term care and disease management programs.
• Develop individualized case plans and coordinate with providers and care teams.
• Communicate updates with providers, physicians, pharmacists and care management staff.
• Identify opportunities to ensure members receive all necessary care under their benefit plan.
• Provide information and education on social issues, mental health, and available services.
• Develop programs to reduce admissions and improve health outcomes.
• Coach junior colleagues in social work techniques and processes.
• Collaborate with community members and policymakers to address issues.
