CVS Health is seeking a Care Coordinator to manage members with long-term rehabilitation and psychosocial needs, coordinating care across the continuum to prevent readmissions and optimize quality of life.

This remote role requires assessments, individualized care planning, and collaboration with providers and community resources. Travel up to 50% within Wayne and Macomb Counties; 40-hour work week.

Competenze

• Bachelor's degree in Social Work, required by state law.
• Must be willing to travel 25–50% of the time in Wayne and Macomb Counties.
• Reliable transportation required.
• Basic problem-solving and decision-making skills.
• Basic digital literacy skills.
• Basic knowledge of medical terminology.
• Ability to deal tactfully with customers and community.

• Perform initial and periodic assessments for Long-Term Care or case management programs.
