Qualifications

• Bachelor's degree in Social Work is required by state law.
• LLBSW / LBSW / LLMSW licensure as applicable.
• Minimum 2 years of medical social work or case management experience.
• Willingness to travel up to 25–50% in specified counties.
• Ability to work in clinical settings with diverse populations.
• Strong problem solving, decision making and digital literacy.

Responsibilities

• Coordinate care for members with long-term rehabilitation and psychosocial needs.
• Implement and monitor case management and quality of life activities.
• Develop individualized case plans and coordinate services across care continuums.
• Communicate with providers, physicians, pharmacists and care teams.
• Identify opportunities to ensure care within the member's benefit plan.
• Provide information and education on social issues and available services.
• Design complex programs to reduce admissions and improve outcomes.
• Mentor junior colleagues in social work techniques and processes.
• Collaborate with community members and organizations to address issues.
