Job summary

CVS Health is seeking a Care Coordinator to support members with long‑term rehabilitation and psychosocial needs. This full‑time, work‑from‑home role coordinates case management across care settings with 25–50% travel in Wayne and Macomb Counties.

You will assess, plan, implement and monitor services, collaborate with providers, and educate on resources to improve outcomes and reduce readmissions. A BS in Social Work and relevant licensure are required.

Qualifications

• Bachelor’s Degree in Social Work required by state law.
• LLBSW, LBSW, LLMSW licensure required.
• Minimum 2 years experience in medical social work or case management.
• Willing to travel up to 25-50% in Wayne and Macomb Counties; reliable transportation.

Responsibilities

• Coordinate care management for members with acute or chronic conditions.
• Assess, plan and monitor care across case or disease management programs.
• Communicate with providers, primary care physicians and care management staff.
• Identify opportunities to ensure members receive required care under their benefit plan.
• Provide information and education on social issues, mental health, and available services.
• Develop programs to reduce hospital admissions and improve health outcomes.
• Coach junior colleagues in social work techniques and processes.
