Qualifications

• Bachelor's Degree in Social Work required by state law.
• LLBSW, LBSW, LLMSW required.
• Willingness to travel up to 25%–50% in Wayne and Macomb Counties; reliable transportation.
• Basic knowledge of medical terminology and problem solving.

Responsibilities

• Controls comprehensive management of members with acute or chronic conditions, focusing on quality of care, outcomes and cost reduction.
• Implements assessments for members in Long-Term Care/case management programs.
• Develops individualized case plans for at-risk members.
• Communicates with providers and care management staff regarding patient status.
• Identifies opportunities to ensure members receive care under their benefit plan in home or community settings.
• Provides information and education on social issues, mental health and available services.
• Develops programs to reduce hospital admissions and improve health outcomes.
• Coaches junior colleagues in social work techniques and processes.
• Collaborates with community members, organizations, and policymakers.
