This role requires collaboration with providers, travel up to 50% within Wayne and Macomb Counties, and a commitment to reducing readmissions while delivering compassionate, compliant care.

Qualifications

• Minimum 2 years experience in medical social work or case management.
• Complies with all state requirements in the state where job duties are performed.
• Must be willing and able to travel up to 25%-50% of the time in Wayne and Macomb Counties. Reliable transportation required.
• Basic awareness of problem solving and decision making skills.
• Basic awareness of digital literacy skills.
• Basic knowledge of medical terminology.

Responsibilities

• Controls comprehensive management of members with acute or chronic conditions, focusing on quality of care, outcomes and reducing costs.
• Implements initial and periodic assessments for the Long-Term Care Program or case/disease management programs.
• Applies case management concepts to develop individualized plans for at‑risk members.
• Communicates updates with providers, primary care physicians, Medical Directors, pharmacists, and care management staff.
• Identifies opportunities to ensure members receive care under their benefit plan in home or community settings.
• Provides information, resources, and education on social issues and mental health.
• Develops programs to reduce admissions and improve health outcomes.
• Coaches junior colleagues in social work techniques and processes.
• Collaborates with community members and policymakers to address issues.
