Job summary

Beatrice Loving Heart is seeking passionate Support Planners to provide essential support and care coordination for individuals in the community. This role helps clients achieve independence, access resources, and live optimally.

You will manage a caseload, collaborate with Social Workers and Registered Nurses, and conduct telephonic care coordination and on-site visits as needed. Flexible telework from home is available, with on-site visits in Maryland.

Qualifications

• Bachelor’s degree in health/human services or related field.
• Strong oral and written communication skills.
• Proficiency in Microsoft Office Suite and computer-based research.

Responsibilities

• Assess clients' needs to develop person-centered Plans of Service.
• Primary contact for clients, conducting telephonic care coordination and information gathering.
• Manage a caseload of up to 55 Clients.
• Coordinate services with families and caregivers.
