Job summary

Beatrice Loving Heart in Maryland is seeking passionate Support Planners to provide essential care coordination for individuals in the community. This role helps clients achieve independence, access resources, and live optimally, ideal for those expanding case management experience.

You will assess needs, serve as primary contact, manage a caseload up to 55 clients, and collaborate with Social Workers and RNs to address complex needs while upholding our values.

Qualifications

• Bachelor’s degree in health/human services or related field.
• Strong oral and written communication skills.
• Proficiency in Microsoft Office Suite and computer-based research.
• Valid Maryland driver’s license and reliable transportation.
• Ability to manage multiple responsibilities independently.

Responsibilities

• Assess clients and develop person-centered Plans of Service.
• Serve as primary contact for telephonic care coordination and monitoring.
• Manage caseload of up to 55 clients.
• Coordinate with social workers, RNs, and agencies to address needs.
• Document activities per billing guidelines and handle priorities.
• Uphold organizational values and ethical standards.
