Job summary

Beatrice Loving Heart is seeking a qualified Team Manager to supervise and coordinate care for clients in Maryland. This fully remote role includes training via Zoom during the COVID-19 period and an on‑going emphasis on care coordination with diverse professionals.

The position requires a Master’s degree in health/human services or related fields and at least three years of case management experience, with strong communication and organizational skills.

Qualifications

• Master’s degree in health/human services or related fields.
• Minimum of three years experience in case management.
• Must possess oral and written communication skills.
• Ability to use Microsoft Office, Excel, and PowerPoint.
• Ability to work with little to no supervision, manage own schedule, prioritize and perform multiple tasks.

Responsibilities

• Ensure team members perform at an optimal level.
• Coordinate and work with Training Team to train new hires.
• Evaluate clients' capacities to develop plans of service for coordination.
• Promote positive health behaviors to support well-being.
• Telework from home with high‑speed Internet and MS Office familiarity.
• On‑site visits to assist with plan of service and care coordination.
• Gather information to develop plans of service for clients.
• Manage multiple priorities and tasks with flexibility.
• Collaborate with Social Workers and RNs to assist with complex needs.
