This hybrid role blends remote work with essential in-person client visits, ideal for professionals in human services, social work, or case management.

You will assess clients, arrange services, document case notes, and support waiver eligibility while promoting independence and safety for clients.

Qualifications

• Bachelor’s degree in Human Services, Social Work, Psychology, or related field.
• Strong written documentation and communication skills.
• Proficiency in Microsoft Office (Word, Excel, Outlook).
• Authorized to work in the U.S.

Responsibilities

• Conduct client assessments and develop Individualized Service Plans (ISP).
• Coordinate HCBS, Medicaid Waiver, and community-based services.
• Serve as a client advocate to ensure health, safety, and independence.
• Connect clients with local providers and community resources.
• Monitor service delivery and ensure compliance with care plans.
• Maintain accurate documentation and case notes.
• Assist with waiver eligibility and renewals.
• Resolve service issues and support client goals.
