Job summary

AstraZeneca is seeking a remote Case Manager to support patients in CST hours with occasional travel. You will be the primary contact for onboarding, ongoing coordination, and adherence support across the patient journey, collaborating with internal teams and external partners.

In this high-touch role, you will educate patients, coordinate injection training, manage BI results, and document interactions to ensure timely access to therapies while maintaining data integrity in the CRM.

Qualifications

• Associates Degree required; background in counseling, social work, nursing, advocacy or sciences is a plus.
• 3–5 years in case management or equivalent experience in healthcare.
• Familiarity with insurance coverage and reimbursement processes.

Responsibilities

• Provide initial and ongoing patient interactions to address questions and support therapy integration.
• Onboard patients, coordinate injection training, and assist with home or virtual sessions.
• Maintain documentation and report adverse events in required systems.
