Job summary

AstraZeneca plc is seeking a dedicated Case Manager to support patients in the rare disease journey from onboarding through adherence. This remote role requires CST time coordination, occasional travel, and collaboration with internal and external partners to ensure timely access and education.

You will handle high-touch patient support, initiate welcome calls, share education materials, coordinate injection training, and maintain meticulous documentation.

Qualifications

• Associate degree or equivalent required.
• 3–5 years case management or equivalent experience (case management, nursing, social service support, insurance reimbursement, patient advocacy, etc.).
• Working knowledge of insurance coverage and reimbursement processes.
• Familiarity with pharmaceutical industry regulations and PV considerations.
• Strong communication and problem-solving skills; proficient data entry and computer use.

Responsibilities

• Provide high-touch patient support and navigation throughout onboarding and ongoing treatment.
• Serve as primary point of contact for case management and onboarding activities.
• Place Welcome Calls and connect patients with education materials and advocacy groups.
• Coordinate injection training and infusion support when needed; maintain follow-up after training.
• Document interactions meticulously to maintain patient case visibility and resolve issues.
