Job summary

Fraser is seeking dedicated Case Managers to support individuals on CADI/BI & DD waivers in Minnesota. You’ll advocate for clients, connect them to local resources, and document case notes with care.

Ideal candidates have a relevant degree, compassion for those with developmental disabilities, and a commitment to inclusion. Role involves serving mostly adults, managing caseloads, traveling to meet clients 2-3 times per week, and remote work after onboarding.

Qualifications

• Bachelor's Degree and licensure as applicable.
• One year of DD experience or Relias I/DD course required.
• Experience or strong interest with adults with serious mental illness.
• Commitment to diversity, inclusion, and culturally responsive practice.
• Valid Minnesota Driver’s License with acceptable record.
• Ability to pass DHS background study.

Responsibilities

• Provide client-centered case management to CADI/BI waiver participants; caseloads ~35; meet 2–3 times per year.
• Provide case management to DD services clients; caseloads ~43; meet 2–3 times per year.
• Advocate for Fraser clients and connect them to community resources; document diligently.
