Job summary

Meridian Services is hiring a Case Manager / Social Worker to support clients across Minnesota from a remote Stearns County location. You will coordinate resources, advocate for individuals with disabilities and elderly needs, and travel as required for meetings and visits.

The role emphasizes serving persons with intellectual disabilities, mental health diagnoses, and aging needs, while offering a hybrid schedule and opportunities for higher pay with advanced degrees.

Qualifications

• Be a licensed Social Worker or licensed Graduate Social Worker per MN Board.
• Bachelor or Master degree as specified; or a 4-year degree in related fields.
• Successful clearance of DHS background check and driving record per policy.

Responsibilities

• Visits with persons on the caseload and attending team meetings.
• Completing or attending meetings on needs assessment; CSSP development.
• Assisting with planning for new service development.
• Locating residential, vocational, and other needed services.
• Monitoring service delivery and ensuring health and safety needs are met.
• Completion of referrals and crisis intervention as needed.
• Developing waiver budgets.
• Completing case notes.
• Acting as an advocate for people we provide services to.
