Job summary

Fraser is seeking a Case Manager to support individuals on the CADI/BI and DD waivers in the Twin Cities area. The role involves advocacy, care coordination, and helping clients access community resources, with a strong emphasis on documentation and inclusion.

Starting pay ranges from $50,000 to $52,000 per year plus a $1,500 hiring bonus. Fully remote after 3–6 months of onboarding; candidates must reside in Minnesota / Twin Cities area.

Qualifications

• Bachelor’s degree in Social Work or closely related field.
• One year of experience in education or treatment of persons with developmental disabilities or related conditions (QDDP qualified).
• Completion of at least one course focused on developmental disabilities.

Responsibilities

• Providing care coordination, monitoring client health and safety needs, and ensuring services meet client goals.
• Intervening to support clients and resolve service gaps.
• Serving as a consistent advocate and connection to resources.
