Job summary

Reach for Resources in Minnesota seeks a case manager to provide waiver services for CADI/BI eligible adults and minors. You will coordinate supports to meet basic needs and help clients access health, educational, and vocational services within a person-centered approach.

The role requires a bachelor’s degree in social work or a related field, plus experience in social services; you will work with a team to maintain high professional standards and ensure confidentiality.

Qualifications

• Bachelor's degree in social work or related field, or equivalent experience.
• Experience or education in social services with case management.
• Ability to meet DHS CADI/BI qualifications.

Responsibilities

• Provide waiver case management to adults or minors under CADI/BI programs.
• Coordinate services and assist individuals to pursue goals.
