Fraser in Minnesota is seeking dedicated Case Managers to support individuals with disabilities through CADI/BI and DD waivers. You will advocate, connect clients to community resources, and maintain precise case notes.

Ideal candidates have a social services background, strong organization, and compassion for diverse populations. Travel is required, meetings 2–3 times per year, with fully remote work after 3–6 months of onboarding.

Qualifikationen

• Bachelor's Degree in a field related to education or treatment of people with developmental disabilities or related conditions.
• Licensure required if degree is in social work.
• At least 1 year of experience with DD populations or willingness to take Relias I/DD course (Fraser covers cost).
• Experience or strong interest in working with adults with serious and persistent mental illness.
• Commitment to diversity, multiculturalism, and inclusion, with culturally responsive practice.

Aufgaben

• Provide client-centered case management for CADI/BI & DD waiver services; caseloads typically ~35 clients for CADI/BI and ~43 for DD, with 2–3 meetings per year.
• Advocate for Fraser clients and connect them with community resources; maintain thorough case notes and documentation.
