Job summary

Meridian Services Incorporated is seeking a Case Manager / Social Worker to provide case management for individuals with disabilities and elderly clients across Minnesota. You will identify resources, help arrange services, and advocate for clients while meeting regulatory requirements.

The role involves travel within the state, meeting clients, and developing service plans, with a hybrid work schedule and potential for higher pay with a Bachelor's or Master’s in Social Work.

Qualifications

• Licensed Social Worker (BSW licensure required for licensure)
• Licensed Graduate Social Worker (MSW licensure required for licensure) per MN Board
• Or a 4-year degree in Human Services, Psychology or Sociology or related field

Responsibilities

• Visits with caseload and attend team meetings.
• Assists with needs assessment and development of individual service plans.
• Locates residential, vocational and other needed services based on needs.
• Monitors service delivery to ensure health and safety of each person.
• Completes referrals and crisis interventions as needed.
• Develops waiver budgets.
• Completes case notes.
• Advocates for the people served.
