Job summary

Fraser is seeking experienced Case Managers to support individuals with CADI/BI and DD waivers in Minnesota. You will advocate for clients, connect them with community resources, and maintain thorough case notes. The role emphasizes compassionate, organized service delivery and culturally responsive practice.

Qualifications

• Bachelor's Degree in social work, special education, psychology, nursing, human services or related field.
• If degree is in social work, social work licensure is required.
• One year of experience with DD populations or contingent on Relias I/DD course.
• Experience or strong interest in working with adults with serious mental illness.
• Commitment to diversity, multiculturalism and inclusion with culturally responsive practice.

Responsibilities

• Provide client-centered case management for CADI/BI and DD waivers.
• Meet with clients 2–3 times per year; manage caseloads (35–43 clients typical).
• Advocate for Fraser clients and connect to community resources.
