Job summary

Fraser is seeking Case Managers to support individuals on CADI/BI & DD waivers in Minnesota. You will advocate for clients and connect them to community resources, with a focus on compassionate, organized care.

The role involves managing a caseload, documenting thoroughly, and meeting clients 2–3 times per year. Requires a bachelor's degree and relevant licensure, plus a valid MN driver's license.

Qualifications

• Bachelor's degree in a related field and licensure as required.
• Experience with DD populations or completion of Relias I/DD course required.
• Commitment to diversity, inclusion, and culturally responsive practice.
• Valid Minnesota driver’s license with acceptable record and ability to pass DHS background study.

Responsibilities

• Provide client-centered case management to CADI/BI & DD waiver clients.
• Maintain caseloads and document case notes accurately; meet clients 2–3 times per year.
• Advocate for Fraser clients and connect them to community resources and services.
• Travel within Ramsey County and surrounding areas as needed.
