Job summary

Meridian Services is seeking a Case Manager / Social Worker to connect clients with resources and services that fit their needs. You will advocate for persons with disabilities and the elderly while managing a caseload of individuals with intellectual disabilities, traumatic brain injuries, or mental health diagnoses.

This remote role requires MN residency for consideration, with travel across Minnesota for meetings; salary starts at $51,000 plus benefits and a hybrid schedule, and higher pay is

Qualifications

• Be a licensed Social Worker or licensed Graduate Social Worker as per Minnesota Board of Social Work.
• Bachelor of Social Work licensure required or Master’s for licensure.
• Alternative: 4-year degree in Human Services, Psychology or Sociology.

Responsibilities

• Visit caseload clients and attend team meetings.
• Develop needs assessments and individual service plans.
• Locate residential, vocational and other needed services.
• Monitor service delivery and health and safety needs.
• Complete referrals and crisis interventions as needed.
• Develop waiver budgets and maintain case notes.
• Advocate for people receiving services.
