Job summary

CVS Health is hiring a Case Management Coordinator to support members with Medicare/Medicaid needs. You’ll help schedule appointments, identify benefits and educate members using care management tools to improve outcomes.

You will evaluate member needs, coordinate care plans, and escalate quality-of-care issues. Travel in Will County and surrounding areas is required, with a remote-hybrid work setup and competitive benefits.

Qualifications

• 2+ years of experience in behavioral health, social services or related field.
• 2+ years of electronic documentation experience and Microsoft Office applications.

Responsibilities

• Evaluate member needs and eligibility; recommend care plan actions.
• Coordinate and monitor assigned care plan activities.
• Collaborate with case managers, providers and health programs.
• Engage in team meetings and support care decisions with documentation.
