Based in Illinois with remote work options, the role requires travel up to 75% from your home location in the Chicago/Northwest side area and a RN license.

Qualifications

Must reside in the state of Illinois.
Willingness to travel up to 75% from home location in/around Chicago/Northwest side.
Minimum 3–5 years clinical practical experience.
Independent thinker with strong analytical and problem‑solving skills.
Proficient with MS Word, Excel, Outlook and PowerPoint; familiarity with proprietary applications.

Responsibilities

Coordinate case management activities and assist with appointment scheduling.
Identify benefits and education resources for members through care management tools.
