Responsibilities

• Evaluate members: assess needs/eligibility and recommend care plan.
• Identify high risk factors and arrange referrals to care management or crisis intervention.
• Coordinate and monitor care plan activities and progress.
• Consult with medical directors and case managers to improve quality of care.
• Document care processes in compliance with regulations.
