Job summary

CVS Health Corporation is seeking a Case Management Coordinator to support the case management process and help members access benefits and education through care management tools. This role requires an Illinois residence and the ability to travel up to 50-75% around Chicago/South Side and surrounding areas.

The ideal candidate has 3–5 years of clinical experience, strong analytical skills, and comfort working remotely using collaboration tools to connect with teams and patients.

Qualifications

• Must reside in the state of Illinois.
• Must have reliable transportation and be willing to travel up to 50-75% of the time from home location in and around Chicago/South Side and surrounding areas, IL.
• Minimum 3-5 years clinical practical experience.
• Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually
• Excellent analytical and problem-solving skills
• Effective communications, organizational, and interpersonal skills.
• Ability to work independently
• Effective computer skills including navigating multiple systems and keyboarding
• Demonstrates proficiency with MS Word, Excel, Outlook, and PowerPoint, as well as some proprietary applications

• Assist with appointment scheduling and accessing benefits and education for members.
