Qualifications

• Bachelor's or higher degree in Behavioral Health or Human Services.
• Minimum 2+ years in Behavioral Health or Social Services.
• Experience with electronic documentation and MS Office.
• Willingness to travel 50-75% around Chicago, IL.

Responsibilities

• Evaluate members' needs and eligibility and propose a care plan.
• Coordinate and monitor assigned care plan activities.
• Escalate quality of care issues and support provider collaboration.
• Coach members to engage in decisions and healthy behaviors.
