You will evaluate member needs, coordinate care plans, liaise with clinicians, and ensure compliance with regulatory guidelines.

Qualifications

• 2+ years experience in Behavioral Health, Social Services or related field.
• Willingness to travel 50-75% in Illinois (Lake, McHenry, Northern Cook Counties).
• Reliable transportation; mileage reimbursed per policy.
• 2+ years of electronic documentation experience and MS Office skills.

Responsibilities

• Evaluate members’ needs/eligibility and recommend case resolution options.
• Identify high risk factors and appropriate referrals.
• Coordinate and monitor care plan activities and progress.
• Collaborate with clinicians and other health programs to improve outcomes.
• Document care and ensure regulatory compliance.
