• 2+ years experience in Behavioral Health, Social Services or related field.
• Must be willing and able to travel 50-75% around Chicago, Illinois.
• 2+ years of electronic documentation experience and Microsoft Office applications.

Responsibilities

• Evaluate members' needs/eligibility and recommend care plan actions.
• Identify high risk factors and coordinate referrals to clinical case management.
• Coordinate and monitor assigned care plan activities and progress.
• Support quality of care through collaboration with medical directors and care teams.
• Mentor peers and participate in team meetings to improve engagement and outcomes.
