• Must be willing and able to travel 50-75% in the Metro East, Illinois area.
• Must have 2+ years of electronic documentation experience and Microsoft Office applications.

Responsibilities

• Evaluate members’ needs and eligibility and propose care plan approaches.
• Identify high risk factors and escalate as needed.
• Coordinate and monitor assigned care plan activities.
• Collaborate with case managers and physicians to resolve barriers to care.
• Escalate quality of care issues through established channels.
• Secure appropriate options and services to meet member needs.
