CVS Health is seeking a Case Management Coordinator to support member care through scheduling, benefit education and accessing resources. This full-time remote role requires 50-75% travel in the Metro East/Southern Illinois region and standard business hours.

The role focuses on evaluating member needs, coordinating care plans, and collaborating with case managers and medical directors to optimize outcomes. A Bachelor's or master-level behavioral health qualification is required.

Qualifications

• 2+ years experience in Behavioral Health, Social Services or related field equivalent to program focus.
• Willing to travel 50-75% of the time in the Metro East, Illinois area.
• 2+ years of electronic documentation experience and Microsoft Office applications.

Responsibilities

• Evaluate members' needs/eligibility and recommend care resolution and benefits options.
• Identify high-risk factors and coordinate appropriate referrals to clinical case management.
• Coordinate and monitor assigned care plan activities and progress.
• Support care quality and outcomes by collaborating with medical directors and other programs.
• Identify and escalate quality of care issues through established channels.
• Use negotiation to secure options and services meeting member needs.
