Qualifications

• 2+ years experience in Behavioral Health or related field.
• Willing to travel 50-75% around Cook County, IL.
• Reliable transportation and mileage reimbursement.
• Experience with electronic documentation and MS Office.

Responsibilities

• Evaluate member needs and eligibility and coordinate care plans.
• Assist with scheduling appointments and accessing benefits.
• Collaborate with case managers and healthcare providers to improve outcomes.
