• Must have 2+ years of experience with electronic documentation and Microsoft Office applications.

Responsibilities

• Evaluate members' needs and eligibility and recommend case resolution strategies.
• Identify high risk factors and coordinate care plan activities and referrals.
• Coordinate and monitor progress of assigned care plans.
