Job summary

CVS Health is seeking a Case Management Coordinator to support the DSNP program in a full-time remote role across the United States. The schedule is Monday–Friday, 8:00 a.m. to 5:00 p.m. EST, with a base pay range of $21.10–$40.90 per hour and eligibility for a bonus program.

The coordinator will evaluate member needs, assist with scheduling, and educate on benefits while collaborating with care managers, providers, and community resources to improve outcomes.

Qualifications

• 2+ years in behavioral health or social services.
• Ability to work 8:00a-5:00p EST.
• 2+ years of experience with Microsoft Office applications (Word, Excel, Outlook).

Responsibilities

• Coordinate and monitor care plans for referred DSNP members.
• Assist with appointment scheduling and education on benefits.
• Identify risk factors and refer to clinical case management as needed.
• Collaborate with case managers, providers, and other health programs.
