Job summary

CVS Health is seeking a remote Case Management Coordinator to support DSNP members nationwide. You will collaborate with members, providers, and community programs to ensure access to benefits and education while coordinating care plans and scheduling.

The role emphasizes critical thinking, advocacy, and outcomes-focused care. The position requires at least 2 years in behavioral health or social services, and proficiency with Microsoft Office.

Qualifications

• 2+ years experience in behavioral health or social services.
• Must be able to work 8:00a-5:00p EST.
• 2+ years of experience with Microsoft Office applications (Word, Excel, Outlook)

Responsibilities

• Evaluate referred member needs/eligibility and recommend care plan approaches.
• Identify high risk factors and coordinate appropriate referrals.
• Coordinate and monitor the implementation of care plan activities.
• Escalate quality of care issues as needed and participate in case conferences.
• Provide coaching and support to empower members to participate in their healthcare decisions.
