Job summary

CVS Health Corporation seeks an experienced RN Case Manager to provide telephonic case management for members with complex healthcare needs. This remote role involves assessing, planning, coordinating, and managing care to improve health outcomes.

The successful candidate will work Mon-Fri within standard business hours, 7:00 AM–6:00 PM CST, with flexible lunch options and no nights or weekends currently required. Competitive pay and benefits apply.

Qualifications

• Active, unrestricted RN license in state of residence.
• Willingness to obtain a Compact RN license (company-paid).
• Minimum 5 years of clinical RN experience.
• Associate Degree in Nursing (ADN).
• Ability to work within Monday–Friday business hours.

Responsibilities

• Conduct telephonic assessments and develop individualized care plans.
• Identify health risks, barriers, and opportunities to improve member wellness.
• Coordinate care, referrals, and transitions to appropriate clinical programs and resources.
• Apply clinical judgment to support members with complex and co-morbid conditions.
• Collaborate with multidisciplinary teams to achieve optimal member outcomes.
• Maintain compliance with regulatory requirements and company policies.
