Job summary

CVS Health Corporation is seeking a Case Management Coordinator for a full-time telework role open to candidates in the United States. You will help elevate patient care for Dual Eligible Special Needs Plan (DSNP) members by coordinating scheduling, benefits education, and access to services across a broad range of programs.

Responsibilities include evaluating needs, coordinating care plans, and collaborating with care teams to achieve optimal health outcomes.

Qualifications

• 2+ years experience in behavioral health or social services.
• Must be able to work 8:00a-5:00p EST.
• 2+ years of experience with Microsoft Office applications (Word, Excel, Outlook).
• Bachelor's degree in behavioral health or related field preferred.

Responsibilities

• Evaluate member needs and eligibility to inform care planning.
• Identify high risk factors and coordinate appropriate referrals.
• Coordinate and implement care plan activities and monitor progress.
• Collaborate with providers, case managers, and programs to improve outcomes.
• Escalate quality of care issues and secure appropriate services.
• Provide coaching and information to empower members in healthcare decisions.
