Qualifications
- Active, unrestricted RN license in state of residence.
- Willingness to obtain Compact RN license (company-paid).
- Minimum 5 years of clinical RN experience.
- Associate Degree in Nursing (ADN).
- Ability to work within Monday-Friday business hours.
Responsibilities
- Conduct telephonic case management with focus on complex healthcare needs.
- Assess, plan, coordinate, and manage care for members.
- Navigate resources and support optimal health outcomes.
- Collaborate with multidisciplinary teams to ensure continuity of care.
- Maintain compliance with regulatory requirements and company policies.
- Document care plans and track progress in medical records.
