Job summary

CVS Health Corporation is seeking a remote Case Manager RN to assess and coordinate care for members of our Federal Plans. This 100% remote role requires a clinical background, strong communication and data-driven decision making to promote wellness and appropriate resource use.

You will conduct comprehensive assessments, develop care plans, advocate for coverage, and collaborate with healthcare providers to improve outcomes. Normal hours are Mon-Fri, 8:00am-5:30pm AZ time.

Qualifications

• Active, current RN license in state of residence
• Ability to be licensed in all non-compact states
• Willingness to work Monday–Friday, 8h shift 8am–5:30pm Arizona time

Responsibilities

• Telephonically assess and coordinate case management activities for members
• Develop proactive care plans to address short- and long-term health outcomes
• Advocate for patients to maximize healthcare coverage and resources
• Conduct comprehensive clinical assessments and identify needs
• Utilize data-driven methods to tailor case management programs
• Promote quality, cost-effective outcomes and program efficiency
• Identify and escalate quality-of-care issues
• Assess health literacy and technology capabilities of members
• Employ motivational interviewing to engage members in behavior changes
