Qualifications

• Bachelor\'s degree in health and human services required.
• 2+ years of relevant experience.
• TB screening program participation required.
• Valid state driver\'s license and vehicle insurance.
• Home office must have a private room with a locked door.

Responsibilities

• Support ongoing member transitions in Indiana Medicaid programs and related enrollments.
• Complete transitions and assist with planning and follow-up care.
• Collaborate with Member Advocate Coordinator and other plan departments.
• Ensure transfer/receipt of prior authorization decisions and clinical data.
• Conduct telephonic and in-person meetings within assigned region.
• Assess member needs to establish a member-specific care plan and coordinate services.
