Job summary

Humana Inc’s CenterWell Home Health is seeking a Telephonic Care Coach to join a remote team supporting member wellness through coordinated services and resources. The role reports to the Manager, Care Management and focuses on removing barriers to care and monitoring progress.

Requirements include 2+ years of related experience, proficiency with MS Office, strong communication, and a health/ human services background; travel may be required occasionally.

Qualifications

• 2+ years of related experience.
• Proficiency with Microsoft Office Programs: Outlook, Word, Excel, SharePoint and Teams.
• Ability to use a variety of electronic information applications/software programs.
• Exceptional verbal/written communication and interpersonal skills.
• Must be passionate about contributing to an organization focused on continuously improving consumer experiences.

Responsibilities

• Employ a variety of strategies, approaches and techniques to support a member's optimal wellness state by coordinating services & resources.
• Identify and resolve barriers that hinder effective care.
• Ensure patient is progressing towards desired outcomes by continuously monitoring patient care through use of assessment, data, and conversations with member.
• Understand own work area professional concepts/standards, regulations, strategies and operating standards.
• Make decisions regarding own work approach/priorities, and follows direction.
• Work is managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation.
