Job summary

Humana is seeking a Transition Coordinator (Care Coach 2) to evaluate member needs and guide transitions through Indiana Medicaid and related care settings. You will collaborate with the Member Advocate Coordinator and other teams to ensure seamless communication and coordinated care during transitions.

Qualifications

• Bachelor’s degree in health and human services required.
• Minimum 2 years of relevant experience.
• Must have valid state driver’s license and maintain vehicle insurance.
• Must have a separate home office with a locked door.
• Residence in Indiana counties Laporte, St Joseph, Marshall, Elkhart or Kosciusko.

Responsibilities

• Support transitions into Indiana Medicaid programs and care settings.
• Plan and follow up on transitions and post-transition care.
• Collaborate with Member Advocate Coordinator and other teams for coordinated care.
• Transfer and secure prior authorization decisions and clinical information.
• Assist with custodial to home and community transitions.
• Conduct telephonic and in-person stakeholder meetings.
• Assess needs to establish member-specific care plans.
• Ensure transitions to the least restrictive setting to maintain well-being.
• Guide members to resources and coordinate with payers and providers.
