Job summary

Humana Inc. is seeking a Senior Care Coordinator to support Care Coordinators through precepting, coaching, and mentoring while ensuring a patient-focused, compliant field operation.

This remote field role requires substantial local travel (75-90%) to visit members in homes or care facilities. The position emphasizes onboarding, case management, and collaboration with interdisciplinary teams to drive member outcomes and access to resources.

Qualifications

• Bachelor's degree in social sciences, social work, human services, or a related field.
• Unrestricted LPN license in Illinois with one year experience in assessments and provision of formal service for the elderly.
• Licensed Professional Counselor (LPC).
• Licensed Social Worker (LSW).
• Must reside in the State of Illinois.
• 2+ years of experience in health care and/or case management.
• 2+ years with Medicare & Medicaid recipients and/or long-term care &/or Home and Community based services and/or working with managed care organizations.
• Ability to use a variety of electronic information applications/software including EMR.

Responsibilities

• Provides coverage for Care Coordinator 1 and 2 and documents in the clinical system.
• Mentors and supports the team and training initiatives.
• Provides daily reports to Care Coordinators regarding outstanding cases and reviews daily compliance dashboards.
• Supports audit initiatives and internal/external auditing processes.
• Completes field work support and reviews reports.
• Supports new hire onboarding including precepting and case audits.
• Coordinates services and evaluates the care plan against the member's goals.
• Ensures progress towards outcomes through assessment, data, conversations, and active care planning.
• Guides members/families towards resources appropriate for their care.

Education

Bachelor's degree in social sciences, social work, human services, or a related field

Unrestricted Licensed Practical Nurse (LPN) in the state of Illinois with one year experience in assessments

Licensed Professional Counselor (LPC)

Licensed Social Worker (LSW)
