Job summary

Humana is seeking a Member Advocate Coordinator to support ongoing transitions in Indiana Medicaid programs, enrollment, and care settings. You will help prepare for transitions, follow up on care after transitions, and ensure continuity across the plan with member and provider communication.

You will also manage transfer of prior authorization decisions and clinical data. This role combines telephonic and in-person meetings with long-term care members, discharge planners, family members/POA's,

Qualifications

• Bachelor's degree in health and human services field
• 2+ years of relevant experience
• TB screening required
• Valid state driver's license and vehicle liability insurance
• Separate room with a locked door for a home office
• Must reside in Clark, Floyd, Harrison or Scott County (Indiana)

Responsibilities

• Support member transitions in Indiana Medicaid programs, enrollment, and across care settings.
• Coordinate care by reviewing prior authorizations, utilization data, and clinical information.
• Assist transitions from custodial settings to home and community-based care.
